Corporate Registrations Form for Roundtable XXIII 1-3 February 2012

Note Well: This is NOT an individual registration!

Please use this form to to request one or more Corporate Registrations for the upcoming CalConnect Roundtable. Corporate Registration badges have no individual name associated with them and the badge is intended to be passed between two or more individuals to allow one at a time to attend sessions at the Roundtable.

The following restrictions apply to Corporate Registrations:
  1. Only Consortium Members may purchase Corporate Registration badges
  2. You may purchase no more corporate registrations for a Roundtable than you purchase regular (individual) registrations
  3. The Corporate Registration badge may be passed only between legitimate employees or volunteers of your organization
  4. Each such individual must be made aware of the Member Agreement and Code of Conduct governing attendance at and participation in the Roudntable
  5. Corporate Registration badges may not be time-shared or exchanged except between meal and break periods; that is, you may not have individuals coming and going and exchanging badges during sessions

You have a choice of payment options including check or electronic funds transfer, requesting that the Consortium send you an Invoice for payment, or payment by credit card.

This form should be used to request one or more Corporate Registrations for the Roundtable on a single Invoice, and paid via a single transaction.

Please be aware that while an individual wearing a Corporate Registration badge is entitled to meals and breaks served while the individual is in attendance, CalConnect regrets that it is unable to cater to individual dietary restrictions for Corporate Registration badgeholders.

How do you plan to pay?

We do not require an invoice in advance and will send a check or money order, or pay via ACH (electronic funds transfer).
We require an invoice to initiate payment.

Corporate Registration Badge Responsibility: Please provide the name of the individual who will obtain the Corporate Registration badge(s) from us at the beginning of the Roundtable, pay for them if you choose to pay at the event, and be responsible for their use. This individual must be individually registered for the Roundtable.
