Please use this form to register for the upcoming Consortium Roundtable. You have a choice of payment options including check or electronic funds transfer, requesting that the Consortium send you an Invoice for payment, or payment by credit card.

Attendance at Consortium Roundtables is limited to representatives of Consortium Members or organizations attending a single Roundtable as an Observer.

This form should be used to register one or more people for the Roundtable on a single Invoice, and paid via a single transaction. If your colleague(s) require separate invoices or will pay separately, please have them register separately.

Registration Fee

The fee for Roundtable participation is $350 per individual for registrations received before 21 September 2008 and $395 for registrations received on or after 21 September 2008.

How do you plan to pay?

We do not require an invoice in advance and will send a check or money order, or pay via ACH (electronic funds transfer).
We require an invoice to initiate payment.

(Please Note: Our preferred option is for you to pay in advance as it is difficult to arrange the time at the meeting to process credit cards. If you are also registering for the CalConnect Interoperability Test Event(s) and wish to receive a single Invoice or execute a single payment transaction covering both this Roundtable registration and the C.I.T.E. registration, please contact us as indicated in the response you will receive to this submission.


Roundtable Observer Agreement (required for observers)

You only need to check this box if you are attending as an observer. If you represent a member of the consortium your organization has already agreed to the Member Agreement and Code of Conduct.

As observers at this Consortium Roundtable I agree for myself, my organization, and for any other individuals named above for whom I am acting in submitting this registration, that we have read and will abide by and conform to the relevant sections of the Calendaring and Scheduling Consortium Member Agreement and member representative Code of Conduct with regard to attending this Roundtable and subsequent actions resulting from our attendance at the Roundtable as though we were a Consortium member.
